Island Health Care
Wellness Program
What is the IHC Wellness Program?
How to Participate:
The Wellness Program is free for eligible Island Health Care patients. If you’ve been diagnosed with a chronic condition like diabetes, high blood pressure, or high cholesterol—and you’re ready to make some lasting lifestyle changes—we’d love to support you. Ask your Primary Care Provider if the Wellness Program is a good fit for your care plan, or fill out the form at the link below!
The Island Health Care Wellness Program is a free, group-based lifestyle education program for patients managing chronic health conditions like diabetes, high blood pressure, or high cholesterol. Our goal is to make lasting lifestyle changes approachable and sustainable—with the support of trusted providers and a compassionate community.
Each session of the program runs between 6 and 15 weeks and is structured around three core pillars: nutrition, movement, and stress management. Participants meet regularly in small groups to explore how everyday choices—what we eat, how we move, and how we care for our mental health—can dramatically improve overall well-being.
The program is led by a dedicated care team, including IHC’s registered dietitian and public health nurse, with guest experts like IHC medical providers, personal trainers, yoga instructors, and mindfulness coaches joining along the way. Participants receive hands-on education, practical tools, and a safe space to ask questions and connect with others facing similar challenges.
Designed with your whole health in mind, the Wellness Program helps you feel more in control of your condition, more connected to your community, and more confident in your ability to make healthy choices that last.
