Important news
Public Health Nursing Services Transition
As of December 15, 2025, Island Health Care’s contracts for Public Health Nursing (PHN) services with island towns have concluded. PHN services are now provided by the Visiting Nurse Association of Cape Cod (VNA), working in partnership with the Town Boards of Health.
We have notified all current PHN patients about this transition and are coordinating with VNA to ensure continuity of care.
For PHN services including well-baby visits, please contact the VNA of Cape Cod.
NOTE: Due to holidays, vacations, and unforeseen scheduling changes, we recommend you confirm the date and time of each wellness clinic with the appropriate Council on Aging before attending.
Diabetes Support Group
Take control of your health and live well with diabetes. Meet us on the third Monday of each month at 1:00 PM at the Tisbury Senior Center. Our support group offers discussions led by a Registered Nurse, guest speakers, cooking demonstrations, and valuable self-management tools. Whether you’re looking for guidance or community, we’re here to support you.
